Lincolnshire County Council has an exciting opportunity for a Level 2 Community Care Officer within our Hospital Service at Pilgrim Hospital, Boston.

This is an excellent opportunity to work within the hospital discharge and transfer of care service. There has never been a better time to join our team and participate in growing our service to be innovative, creative and providing high-quality support to people when they need it most.

About the Role

We strive to be proactive and use a strengths-based approach, to enable our customers to return home and live as independently as possible. The customer is at the centre of the process, our highest continuing aim is to provide the best service possible to service users and their families.

Working creatively when planning support packages, utilising "outside of the box" thinking and solutions to ensure an individual care plan is in place that empowers the service user to participate.

LCC is leading the way on the introduction of a fully integrated Transfer of Care Hub, to support access to the right services in a more fluid way, reducing delays and delivering benefits.

You will work as part of the 7 day Adult Care rota. This equates to approximately 1 in 4 weekends worked, and these are all scheduled 3 months in advance.
