Contract type: Occasional evening and weekend work as required

The Health Improvement Services have exciting opportunities for a highly motivated and skilled individual to join our successful team as a Community Support Worker. You will be working alongside a committed team of community-based professionals, all with the passion and expertise to deliver a client-focused service; helping to improve lives and promote independence.

You will be responsible for supporting and coaching individuals to navigate through health, community, and voluntary sector services in Swindon to improve their well-being, self-efficacy, and quality of life. You will promote personal resilience, safety, connection, and creativity to ensure the management of their long-term condition(s) and circumstances.

You will be responsible for managing a caseload and will work closely with colleagues to ensure the most suitable support is in place and reviewed accordingly. An element of the Safely Home and Self Neglect and Hoarding roles may require manual handling to support the facilitation of hospital discharges, for which you will receive the suitable training. You will receive ongoing supervision and training as well as peer support from team members.
