We have an exciting position in our Prevention and Engagement team within our Stoke-on-Trent Partnership service. The role will involve working within our prevention and engagement team, completing assessments with service users, as well as providing intervention such as harm reduction and working with the Needle and Syringe Programme.

You will need to be an excellent communicator and good team player. Being able to motivate and engage people is key - as well and competently assess and manage risk.

As a Prevention and Engagement Worker, you will be working in close partnership with other health care professionals, partner agencies, service commissioners, and internal clinical/mental health teams to ensure that service users receive prompt advice and treatment.

A key element of the role will be to assist in the delivery of an integrated service by completing assessments, recovery plans, referrals, support, care coordination and appropriate interventions (e.g. psychosocial, pharmacotherapy, harm reduction).

You'll also be able to confidently and expertly assess service users, carrying out triage and comprehensive assessment, as required and making appropriate onward referrals to specialist agencies.

This is a fixed term role until 30th June 2023. The provisional interview is the 13th July 2022.

Essential Skills

Evidence of continuous professional development
OCN/NVQ level 3 orequivalent or willing to achieve this within 12 months
Demonstrable experience of working in the health, social care, criminal justice or other specialist setting
Successful track record ofworking with substance misuse service users
Knowledge and understanding of health and safety, including assessing high-risk situations for service users
Knowledge and understanding of group Working: - Assessment, care planning and key working
- Harm reduction
- Issues affecting substance misusers
- Rehabilitation
- Solution-focused Brief Therapy
Ability to work as part of a multidisciplinary team
Ability to build effective relationships with a range of internal and external stakeholders
