Registered Nurse and Case Manager- Dual Role
Newcastle / GBP38,000 Per annum + Car Allowance / Some Hybrid Working
Registered Nurse and Case Manager to cover the North East is required.
My client is a forward thinking Group that is a progressive, integrated, specialist Healthcare Provider with a unique focus on complex care.
With hospitals, care homes, residential supported living and 24 hour care at home.
This role is a dual role consisting of part time nurse competency training and support and part time case manager.
Nurse and Case Manager duties include:
* Provide part time clinical competency training and support for our Support Workers and work within the clinical team to identify and prioritise using our training report.
* assess client need, and implement multi-disciplinary support through therapeutic relationships with clients and their families
* promote supportive, open and trusting relationships with support workers.
* Maintain excellent communication and liaison with all professionals including therapists, Social Services/Local Authorities, CCGs, Medical Practitioners, families, Solicitors and Insurers and others as and when necessary
* Maintain excellent levels of communication with all members of CCMS.
* Carry out detailed assessment of the care, rehabilitation and case management needs of clients, with detailed understanding of the clients needs and vulnerabilities for living independently within the community
* prepare Case Management Reports and establish and agree Case Management Plans.
* Monitor progress and changes in the client's situation and review the case management plan accordingly.
* Act as advocate for the client and establish an excellent working relationship with the client and his/her family.
* Liaise with multi-disciplinary professionals as and when required in order to co-ordinate services and sharing of information.
* Implement and monitor an appropriate support regime, providing supervision and training for support workers as required to ensure they are always up to date with the most relevant training and information.
* Resource and procure suitable equipment, services and activities as and when required.
Register Nurse and case Manager Requirements:
* Registered Nurse with Valid Pin
* Excellent knowledge of all Standards and NMC code of practice
* Proven track record of person-centred care planning
* Experienced leader and mentor
* Drive, passion and commitment to deliver excellent results in all areas
* The ability to work alone and in a team
Registered Nurse Position Renumeration:
* GBP38,000 per annum
* Car Allowance of GBP400 per month (after 6 month probation)
* 29 Days Holiday + Bank Holiday
* Work/life balance
