Community Case Manager
Her Majestys Prison Service

Full time 9 am to 5 pm Monday to Friday
Salary range GBP23,840.00 - GBP24891 per annum
CMD Recruitment is working with a national provider to support a vacancy for a case manager on the CF03 programme.
CFO3 is supported by Her Majesty's Prison and Probation Service (HMPPS) and part-funded by the European Social Fund (ESF). It is a voluntary programme designed to help people within the justice system prepare for when they leave custody and re-enter the community.
The programme aims to increase social inclusion by engaging with support from the community in areas such as health, family, housing, education and employment
The case manager works outside the prison setting alongside the probation service with a caseload of ex-offenders who are on license
The case manager supports the service user following a series of pathways including
Debt management
Personal Hygiene
Employment support
Support for training courses
Referral to housing organizations
Provide advice guidance and support to overcome barriers to social
inclusion and signpost to internal and external support
This is a payment by results programme so there are targets around service user progression such as training course enrolment and employment outcomes
