Evolve is working with a leading, rapidly expanding Healthcare Services provider, with a permanent position available for a Band 5 Registered General Nurse to work on a part time basis (2 days per week).
This is a 6-month fixed term contract, where you will be an active member of the Neurology team, facilitating and supporting headache patients in receiving therapy reviews and optimum treatment, as well as assisting in the development of the headache services for patients at the Royal Stoke University Hospital.
Ideal Requirements
* NMC Registered General Nurse
* Preferably you will have an interest or understanding of the management and treatment of chronic headache / migraine.
* Practice knowledge of care planning and reviewing/managing patient pathways.
* Professional, understanding, and patient with a warm personality.
Role Responsibilities
* Support headache patients / facilitate therapy review service.
* Deal directly with patients to answer medication queries and to support them with unscheduled health care contacts and acute headache occurrences.
* Service development in collaboration with CNS and Headache Consultants.
* Assist the department to establish a database with comprehensive patient data to inform the clinical team on treatment choices and management strategies.
* Work with colleagues in the pharmacy department to facilitate the delivery of homecare service support.
* Support the development of a headache management pathway within the patient's electronic patient record (EPR) to introduce effective templates.
* Undertake a review to capture the current processes for the recording of patient headache diaries.
* Work with and support the wider team in developing this process with the aim of moving towards electronic diaries for all patients.
* Provide much needed administrative support to facilitate each patient's initiating therapies.
