Case Co-ordinator - Homelessness 

£23,000 a year 

Stoke-on-Trent, Staffordshire 

Full Time (37.5 hours per week)

The role 

You’ll work with customers to assess their needs and together, design and deliver a service coordination plan that works for them. You’ll help customers access a wide range of services that meet their needs and help them achieve their goals and aspirations.  

Job requirements 

• Carry out customer assessments to establish their risks, needs and requirements. You’ll then coordinate access to a wide range of services that are appropriate to their needs. 
• Develop and maintain an in-depth knowledge of the local services available to people with multiple needs. 
• Jointly design and deliver a service coordination plan for customers to help them achieve their goals and aspirations. 
• Work with customers to set, manage and spend a personal budget to help them achieve their service coordination plan objectives. 
• Work closely with other volunteers and lived experience organisations including Expert Citizens CIC, peer mentors and student volunteers. 

What our client is looking for 

• Educated to GCSE Level or with a good level of maths and English. 
• Experience working with people who have experienced multiple disadvantage and exclusion from services. 
• Experience delivering support to customers and recognising their individual needs. 
• Great communication and time management skills. 
• A genuine understanding of people experiencing homelessness. 
• DBS check. 
• Full UK driving licence and access to own vehicle for work purposes.
