Care Manager

This role is based around the South East working alongside the clients and the carers in the area.. This is a full time position Monday-Friday (37.5hrs per week) and ideal person will hold a full UK driving licence, have access to a vehicle and be happy to work one day in the office which is in the Surrey/Hampshire area.
As the manager, your main role will be to carry out assessments and reviews of client's needs and to produce support plans, alongside providing supervision, training and support to the carers.
Other duties will involve:
Client satisfaction reviews
Carrying out visits to client's homes and hospitals/other settings to carry out initial client assessments
Start-up of new packages of care with ongoing monitoring and review
Completion of Care Plans, Risk Assessments and all other relevant paperwork
Ensure client's needs are met safely in all aspects of their care, not limited to medication, nutrition, mobility
Ensure the client's care is safe by ensuring risk assessments are up to date and robust
Ensure the client's care is effective by ensuring good outcomes are achieved and the client can maintain the best quality of life for their situation
To ensure carers are aware of their role and duties for their clients and to ensure the quality of care provided is of a high standard and where there are concerns that these are dealt with effectively and efficiently
Ensure that Care workers are supported, and routine spot checks, supervisions, observations and competencies are completed regularly
Identify any requirement for additional Care worker training and/or additional competency checks
Liaise with other healthcare professionals and agencies (e.g.: social services) regarding service provision to ensure the needs of the client are met
To have knowledge of safeguarding processes and the ability to deal with such situations in a sensitive and professional manner
To be aware of the mental capacity act and its application
To have working knowledge of CQC regulations and how to apply this in day to day care settings
Any other duties as reasonably required by the Registered Business Manager or a DirectorPerson Specification:
Be an experienced care worker
Be passionate, empathetic and understanding
NVQ Health and Social Care or similar
Have good communication skills
Have mentoring skills for staff supervision
Be able to represent the company at reviews
Reliable, flexible and able to respect confidentiality
Entitled to work in the UK
Able to work alone and as part of a team
Willingness to travel to clients' homes and other sites within the group
