Summary
The Discharge Hub within University Hospitals of Leicester, is currently seeking an enthusiastic, flexible, and motivated Band 6 Specialist Nurse to join our Team You will join a passionate team of specialist discharge sisters whom you will work closely with to achieve safe and timely discharges across the trust. The successful candidate will require excellent interpersonal and communication skills as they will be liaising with patients, family/carers, members of the wider multi-disciplinary team across UHL, working and communicating daily with our System Partners i.e. Adult Social Care and Leicestershire Partnership Trust. Working within a busy team, the successful candidate will also be responsible for completing a wide range of tasks including, ensuring staff members at ward level are aware of discharge arrangements, working with are partners to arrange discharge destinations, ensuring discharge letters/discharge medications are processed in a timely manner, transport is arranged to support patient discharge, and clinical tasks are completed by ward staff to avoid discharge delay. Furthermore, the candidate will be responsible for updating patient records and electronic applications and databases. The post holder will be able to function within the role in a busy working environment, and be able to organise and prioritise their workload to ensure safe and timely discharge. However, the discharge team and wider ward teams are friendly and supportive, and will encourage the post holder to succeed within their role.
