AMAZING PAY RATES GBP10.95 WEEKDAYS AND GBP12.95 WEEKENDS
FULL TIME/PART TIME
MUST BE A CAR DRIVER WITH OWN VEHICLE
National and well known Care Provider is searching for HOSPITAL DISCHARGE CARE ASSISTANTS to assist and support individuals in day to day life in the community and in their own homes.
Contracts available of 20 or 25 or 30 or 35 or 40 or 48 hours per week plus option of overtime. Travelling time payed at full rate. Mileage paid at GBP0.40 per mile.

Pay rates: GBP10.95 weekdays, GBP12.95 weekends (contract time)
Travel time and mileage paid

KEY DUTIES FOR HOSPITAL DISCHARGE CARE ASSISTANTS
Companionship
Support with community outgoings (leisure/shopping trips)
Assistance with some personal care and day to day tasks
Responding to individual's needs in Person Centered Care manner
Assistance with domestic tasks
Reporting and regular communication with the main office team
Support with meals preparation
Assistance with medication
REQUIREMENTS FOR HOSPITAL DISCHARGE CARE ASSISTANTS
Experience NON-ESSENTIAL but desirable
Compassion and Enthusiasm to make change and difference in resident's lives
Able to work independently or as part of a team
Enhanced DBS on the update service or being prepared to undertake one
Flexibility in working hours and ability to work every other weekend
Driving license and access to a vehicle
