- First level registered nurse
- Diploma in health-relatedarea or equivalent experience
- Experience working as part of a Multi-disciplinaryteam/ multi-agency working
- You need to have knowledge and experience of undertaking complex assessments and the planning and evaluation of care package
- Excellent Communication, both written and verbal
- Evidence of effective interpersonal skills
- Clinical Risk assessment skills
- Ability to problem solve and make clinical decisions
- Understanding of NMC standards and implications to practice
- Ability to work independently in community setting
- High clinical standards
- Good IT skills
