Service Support Worker - Support at Home
Location: Greater Grimsby area
Fixed Term Contract until 31st March 2022
35 Hours per week (5 days, mainly Mon - Fri)
Salary: GBP 16,452 per annum (pro rata)

As a Service Support Worker, you will provide the support and care which can make a vital difference to the quality of people's lives, offering practical and emotional support that empower service users and allow them to continue living at home in comfort.

You will respond to referrals from Clinicians and Community Health Professionals and will be required to have the ability to assess service users' needs, complete and follow a support plan for the individual. In addition, you will be providing practical and emotional support in the service user's own home and signposting to other services where appropriate to enable the individual to live independently and reduce the need for hospital admissions.

The service users you're supporting are in the greater Grimsby area, Lincolnshire. This role will work from Diana Princess of Wales Hospital, from our British Red Cross centre at Grimsby (DN33 1HE) and out in the community. This service runs 7 days a week, we envisage the successful candidate to primarily cover the Monday - Friday shifts, between 9am - 5pm. Occasionally you may work at weekends or up to 8pm to help cover colleagues off on annual leave or unplanned absence.

The Independent Living (IL) & Crisis Response (CR) service (IL/CR) supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care and emergency services.

Also, you should be the following:

  • IT literate
  • Full driving licence holder (for over 2 years), with access to your own vehicle if needed. Ability to drive a manual or automatic vehicle.
  • Have a good knowledge of services provided by the NHS and Social Care
  • Be local to the Grimsby (North East Lincolnshire) area
