SOCIAL PRESCRIBING PROGRAMME MANAGER

Salary: £28,000 – £30,000 per annum

Hours: 37.5 hours per week

Accountable to: Chief Executive, Wolseley Community and Economic Development Trust

Staff responsibility: Social Prescribing Link Workers (SPLWs), Health and Wellbeing Coaches, Support staff

Employed by: Wolseley Community and Economic Development Trust
Based: Wolseley Trust offices

This is an exciting opportunity for someone who has experience or understanding of the landscape of social prescribing and personalised care, and an interest in the innovative changes currently taking place in the arena of health and social care nationally.

We are looking for a motivated and dynamic Service Manager with experience of delivering client focused services. The role is responsible for the management, development and operational delivery of the Wolseley Trust’s Healthy Futures Social Prescribing and Personalised Care service, providing a referral system for a network of GP practices across Plymouth.

You will support and inspire a committed and highly responsive team, based in our local communities.
