About the role

The Role of the Community Connector is to enable people living in Camden who no longer use traditional day services to make new community connections and new unpaid relationships. You will do this by using your knowledge of the groups and activities that are available locally to enable people to make informed choices about the groups and activities they want to engage in.

Duties will include:

- Making a difference to the lives of those you support
- Developing new opportunities for community connections and developing lasting friendships and relationships
- Support people to spend their time in ways that give them pleasure and meaning
- Work in a consistent and positive way with local managers and local team members to ensure people get a great service from the whole team
- Support with everyday tasks to maximize the individual's independence
- Work in a consistent and positive way with local managers and other team members to ensure people get a great service
- Develop and maintain links with professional bodies that support Community Connecting

About you

To be a Community Connector at Certitude, the following are essential:

- Experience of supporting people who have learning disabilities to become more active in their community
- Experience of building great relationships which promotes inclusion for people with learning disabilities
- Good knowledge of the London Borough of Camden
- Ability to research new community-based resources and services that meet people's identified needs
- Ability to work on your own initiative and also be a great team player
- Practical, adaptable and organised
- Have interests and skills that they are willing to share and get others involved
- Ability to use technology - PC and modern technology equipment
