Our mission at Response is to enable people of all ages to live a more fulfilled life by providing excellent mental health services, housing, and supported living. Here at Response, we believe that it is possible to recover from a mental health condition, and that the most effective recovery is directed by the person. That is why we are working with Avon and Wiltshire Mental Health NHS Trust Partnership on a pilot project to support adults back into the community from adult acute in-patient hospital wards. Based on the success of this pilot, there could be further opportunities to work across multi-disciplinary teams.

You will work with identified hospital patients prior to hospital discharge to support them to secure suitable accommodation, meet their goals, regain control of their recovery and move towards greater independence. We are looking for Housing and Community Support Workers to join the Adult Service Delivery team at Response. The Adult Service Delivery team focuses on making it easier for those with mental health problems to get the best support when and where they need it. Our main priority for all our roles at Response is to find people that can live our values every day. It is essential that you have experience of working in mental health and housing services and experience working with people with dual diagnosis and homelessness. Experience working with rough sleepers outreach services and experience working in a supervisory role is desirable, but not essential. Knowledge of local housing pathway, housing legislation (homeless reduction act, duty to refer, universal credit), the Mental Health Act, Misuse of Drugs Act 1971 and other relevant legislation is essential to this role. The duties within any role vary from day to day. Some of the core duties within the role will include: Case managing people whose discharge planning is classified as a?delayed transfer of care; Enable the successful placing of those who are at risk of?becoming homeless, or unable to access service that will meet their assess needs; Use specialist knowledge and experience to identify and understand patients' housing issues?to?enable a successful discharge; Supporting relevant staff in the hospital setting to be able to identify?risks?of homelessness; Supporting development of individual discharge plans; Work in partnership with Avon and Wiltshire housing associations?and 3rd?sector organisations?to manage engagement with housing services where required; support patients in accessing personal ID requirements needed to access housing and benefit options; Provide administrative support in making formal referrals to housing services; Connecting rough sleepers to outreach and other services they require; Assessing eligibility for services where the patient has a local connection; To establish housing expertise within the discharge and social work teams at the hospitals; Liaising with landlords, family/friends as appropriate to?broaden?housing options; Providing?signposting?support?to specialist services?in respect of failed asylum seekers; Being responsive?to requests for support via phone and email when not on site.

You will be required to work 37 hours per week on a one year fixed term contract (with the potential of moving permanent), predominantly occurring between 9-5 Monday-Friday but this post will require some level of flexibility to respond to the needs of the patient and availability of professionals who are crucial for achieving the goals of the support plan. Salaries vary depending on the role and experiences. Secondments may be considered if Response have an existing arrangement with the candidates current employer and they are supportive of this arrangement.
