This is an exciting opportunity in a growing diverse sector for an enthusiastic and dynamic Rehabilitation Assistant. The successful candidate will meet the personal and social care needs of patients whilst following specialist treatment support plans devised by the MDT.
The successful candidate will
Will work as part of the multidisciplinary team within Woodlands Rehabilitation Neurological centre.
Deliver daily support and care for patients, including physical health care, personal care, transfer and mobility, eating and drinking, and leisure activity
Adhere to risk assessments and support plans written by nursing and therapy, promoting rehabilitation and person centred care
To maintain timely and accurate clinical notes and records
To contribute to maintain service excellence across all areas
