Project Overview:
“Bridging the Gap” is a 'through the gate' reducing re-offending project, providing mentoring services and group sessions to people released from prison into Lincoln and surrounding areas (within a 35mile radius). We aim to support clients through any issues they have, and to empower them to make positive changes which will impact themselves, their families, and the community around them. We hope that through the encouragement, guidance and often intensive support provided by the mentor, the clients will not reoffend within the first 6 months after release. The project focusses on three referral pathways:
Men released from HMP Lincoln into the City and the surrounding areas (35-mile radius of the City).
Men released from other prisons into Lincoln.
Men & women who have been in custody within the last 2 years who are either working with an agency that is able to refer them on to the programme or through self-referral.
Key Tasks
Mentoring of ex-offenders
In collaboration with partners including the Offender Management Unit, Prison Chaplaincy and Probation, make contact and identify people who may benefit from the Project who are currently in the Criminal Justice System.
Contribute to the multi-disciplinary assessment of client’s needs, formulate and deliver an appropriate support plan.
Support the rehabilitation of people leaving prison, by offering practical support and guidance on a flexible basis matched to the person’s needs.
Adhere to policies and procedures to assess risk in connection with the work.
Maintain contact with clients/beneficiaries to ensure ongoing progression.
Project Development
Develop, alongside the CEO and Bridging the Gap Team Members, a strategic relationship with the Governor and Senior Management Team in order to deliver the Service Level Agreement.
In partnership with Bridging the Gap Team Member, develop and deliver new group sessions.
Community Liaison
Establish and maintain working relationships with Police, Probation Service, Social Services, Health and other statutory agencies in the Lincolnshire area.
Develop and maintain working relationships with all relevant voluntary agencies providing employment, training, education, information services, drug and alcohol advice, and victim support.
Encourage local communities to respond to the needs of people at risk of (re)offending.
Project Monitoring
Provide a regular report and assessment of the Project to the Line Manager, Trustees, and external funders.
Attend monthly reducing reoffending meetings within HMP Lincoln (once appropriate vetting checks have taken place)
Work towards set outcomes outlined within the funding bid.
General and Personal responsibilities:
Work as part of the developmentplus team.
Undertake regular personal supervision with the Line Manager and external Clinical supervision.
Undertake training as identified by developmentplus/yourself.
Undertake other duties as agreed with the Line Manager for the effective delivery of the project aims.
