Our personalisation and community services team aim to provide extensive care and support to individuals and their families, promoting independence and inclusion within their local communities.

 

We have several opportunities, Permanent and Temporary, to join our busy team.

The post holders will act as co-ordinator for the service as required; providing advice, taking detailed notes from families/young people, liaising with local authorities, schools, other professionals and mediators in order to arrange meetings.Report on a range of factors related to the business as it stands now and grows in the future; keep, maintain and update official records relating to staff, people supported, statutory and professional contacts.

The post-holders will be expected to be flexible in their approach and be prepared to undertake other duties when required.
