Position: CFO3 Case Manager
Location: HMP Rochester, Kent
Contract Type: Permanent
Hours: Full Time, 37.5 hours Monday to Friday
Salary: GBP23,000 per annum

Role overview:

The Case Manager will manage a caseload of Prisoners in prison, supporting them to break down barriers to successful resettlement, achieve agreed milestones and enter employment, training or education. Engage eligible Prisoners, provide initial assessment and undertake action planning to prisoners being released from in-scope prisons. Provide advice, guidance, coaching, mentoring and support to a caseload of participants via regular one-to-one meetings and group interventions as appropriate.

Pact's Case Managers support Prisoners in custody to prepare for release and successfully resettle into their local communities. They will support the delivery of the NOMS CFO Programme providing practical help, advice and guidance, as well as motivation and understanding. Providing support for individuals with multiple and complex needs to overcome barriers and progress towards education, training and employment.
