NE Hants & Farnham Social Prescribing Service
21 hrs per week
Grade: 4 (lower) SCP 29 to 33 is £34,717 to £38,736 pro rata
NE Hants & Farnham Social Prescribing Service. Based in Fleet Office.
The Service Manager will support all aspects of the day-to-day running of the Social Prescribing Service to NE Hants and Farnham. They will ensure the service provides effective management of client referrals from participating GPs and other primary care health professionals including holistic assessments, co-designing a social prescription to improve health and well-being outcomes for individuals and a longer-term outcome of reducing the number of clinical/medical interventions required.
The Service Manager will work within a Family Action Quality Assurance Framework and key
responsibilities will include managing and reporting on:
- Performance and information
- Audit and review
- Safeguarding
- Service user feedback and review
For this role, you will be educated to level four or above with a recognised professional qualification in social work, health, education or equivalent. Previous experience of working in a similar role would be desirable.
You will have experience of monitoring information electronically and an ability to review and update data recording systems with a proven ability to develop and maintain effective reporting.
You will have an enthusiasm for delivering a high-quality client service and have a clear understanding of the purpose and aims of the Service.
You will have the skills to lead and motivate a team to deliver to targets and will be passionate about supporting members of the community to achieve consistent and effective outcomes.
