Team Leader - Health Care
Berkeley Home Health are Recruiting and are looking for Team Leaders or Care Managers to join them.
THIS IS A FIELD BASED ROLE
The role involves:
Provide appropriate supervision, training, observations and support to carers
To visit and carry out assessments and reviews of client's needs to produce appropriate support plans
To promote a professional image of the company in dealing with new care packages
To facilitate local service delivery and manage packages of care
Duties:
• Carrying out visits to client's homes and hospitals/other settings to carry out initial client assessments
• Start-up of new packages of care with ongoing monitoring and review
• Completion of Care Plans, Risk Assessments and all other relevant paperwork (e.g. Mar Charts)
• To ensure client's needs are met safely in all aspects of their care, not limited to medication, nutrition, mobility
• To ensure the client's care is safe by ensuring risk assessments are up to date and robust
• To ensure the client's care is effective by ensuring good outcomes are achieved and the client can maintain the best quality of life for their situation
• To ensure the client's care is responsive to their needs
• To ensure carers are aware of their role and duties for their clients and to ensure the quality of care provided is of a high standard and where there are concerns that these are dealt with effectively and efficiently
• Ensure that Care workers are supported, and routine spot checks, supervisions, observations and competencies are completed regularly
• Identify any requirement for additional Care worker training and/or additional competency checks
• Liaise with other healthcare professionals and agencies (e.g.: social services) regarding service provision to ensure the needs of the client are met
• To have knowledge of safeguarding processes and the ability to deal with such situations in a sensitive and professional manner
• To be aware of the mental capacity act and its application
• To have working knowledge of CQC regulations and how to apply this in day to day care settings
Person Specification: NVQ Level 3
• Be an experienced care worker
• Have good communication skills
• Be a trained Risk Assessor
• Have mentoring skills for staff supervision
• Be able to represent the company at reviews
• Reliable, flexible and able to respect confidentiality
• Entitled to work in the UK
• Reliable access to own car
• Able to work alone and as part of a team
Additional Information:
Full driving licence and own car is a requirement.
Willingness to travel to clients' homes and other sites within the group
