Major Healthcare is a specialist recruitment consultancy, providing quality permanent, contract and temporary staffing solutions to clients from our wide network of UK offices.
We are currently looking to recruit a number of District Nurses in Bromley, South East London. As a District Nurse you will deal with a variety of patients in their own homes. You will be responsible for management of a defined caseload of patients and carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of care programmes.
The District Nurse/Senior Community Staff Nurse is a key member of the Integrated Community Team, providing robust clinical services for patients, working closely with allocated GP practices and with members of the team from other professions, putting the patient at the centre of their care.
Key Responsibilities but not restricted too:
* You will provide high quality, holistic, effective community based health services that reflect the needs of the local population, complementing the work of the General Practitioners and Allied Health Professionals.
* Contribute to the clinical delivery of the service. This will include completing holistic patient assessments, designing and reviewing care plans, delivering effective clinical care
* Responsible for contributing to the holistic assessment, planning, delivery and evaluation of patient care.
* Assist with the daily coordination of the team to include daily handover, allocation of work and managing referrals and telephone messages.
* Implementing, monitoring and maintaining our high standards of care.
* Accountable for the proactive management and monitoring of a patient caseload making autonomous decisions about patient care and seeking support from colleagues as appropriate.
* Work in partnership with acute care and social care when required.
* Re-assessing and re-evaluating programmes of care when needed.
* Completing holistic patient assessments, designing and regularly reviewing care plans, delivering effective clinical care.
* Assist with the daily coordination of the team to include daily handover, allocation of work and managing referrals and telephone messages.
* Proactively monitor and manage a daily patient caseload making autonomous decisions about patient care and seeking support from colleagues as appropriate.
* Contribute to the implementation and monitoring of any existing and new patient pathways.
* Be an active member of the team sharing best practice whenever possible.
* Maintain and develop own professional expertise as a clinician.
* Attend and contribute to multidisciplinary meetings, actively participating in interprofessional communication and effective discharge planning.
* Liaise with primary health care team, statutory and voluntary agencies in the provision of care to patients and their families/carers.
* Collaborate with others in the planning and implementation of health promotional strategies and activities.
* Undertake Root Cause Analysis when required and present findings.
* Be aware of all issues relating to Clinical Quality Governance and to meet standards as required.
Successful Candidates
* Valid NMC Pin Number
* District Nurse Qualification
* Full UK Driving Licence and access to own vehicle
* Excellent clinical and communication skills
* Experience working within the a community setting and experience of caseload management
* Ability to support, motivate and lead a team
Salary for this role is up to GBP42,712 depending on experience.
