Care Coordinator - Programme Assistant

Fabulous new job! Our client, a medium sized charity is looking to recruit a care/programme assistant for a newly established role in a new department. This role sits within community services for older people, most with dementia, care and nursing needs. Working alongside the Programme Manager and Community Hub Manager and other team members, you will be responsible for supporting the welfare of service users attending and using the services. You will also be responsible for the frontline coordination of day services delivered to the service users. Community Services, will be delivered both remotely to service users homes and within the community centre building itself.

Some aspects of the role include:

Running and taking part in activities and programming across the community service, in line with agreed standards
Working collaboratively with the programming/coordinator teams to set up appropriate technology to support service users taking part in activities from home.
Carry out actions as indicated on a service-users care plan. This may involve working in collaboration with internal social work colleagues, relatives and external agencies.
Partner with the senior OT - dementia Lead to promote dementia care.
Coordinate and participate in the effective and safe delivery of transport services across the older people services
Day to day operational delivery of culturally services to this religious group of service users
Working with and supporting the manager and the specialisms of community, digital, dementia and programming to ensure that the community services team can effectively share best practice and work in partnership to deliver outstanding services
The skills, qualifications and experience you will need are as follows:-

NVQ level or equivalent experience
Minimum 2 years' experience in a front-line role, working with older and/or disabled people
Minimum 2 years' experience of working with people living with dementia in a personal or professional role
Experience leading a team of volunteers
Experience working in a complex multi stakeholder environment
Experience of or an interest in working in a co-production culture
Knowledge of the Jewish community, across all levels of observance, local parlance, and festivities
Knowledge of The Care Act, The Mental Capacity Act, DOLS and general adult safeguarding best practice
Knowledge of General Data Protection Regulations and their practical application in day to day operational work
Ability to ensure continuously improve its services through rigorous management, monitoring and measurement against business plans, budgets, targets and KPIs
Exceptional customer service skills, with the ability to deliver a consistently high-quality customer experience
Ability to work empathetically and respectfully with people of all ages and abilities
Outstanding administrative, management and organisational skills, with the ability to work within tight deadlines and budgets
Ability to use own initiative and work independently
Ability to handle challenging situations gently, with confidence and integrity
Ability to lead and be led amongst a highly creative and collaborative peer group
Ability to work in a team and build trusting relationships with others
Outstanding verbal and written communication skills and the ability to influence people at all levels, both within and outside the organisation
Excellent problem-solving skills, and able to think clearly under pressure
High levels of IT literacy in the use of modern business systems e.g., Microsoft Office Suite, Internet and cloud-based services and Customer Relationship Management (CRM) systems
