We are looking for a strong Coordinator with excellent management skills to join our client in Chelmsford on a 3 month temporary contract.

Main Purpose

•As the Core Team Coordinator you will work within a multi-disciplinary team providing coordination and administrative support to the Core Team and the patients and their families that they care for and support

•To support Core Team leads in delivering a high-quality service i.e. maintaining audit and mandatory training records, risk assessments and other compliance functions.

•Through telephone, virtual and face-to-face discussions, provide information to patients with palliative and end of life care needs, supporting their families and carers, liaising with Core Team professionals and referring on as necessary to appropriate health and social care partners

•To maintain effective communication skills to deliver information clearly, sensitively and unambiguously with staff, patients, families and professionals by face-to-face, telephone and/or electronic means.

•To support the maintenance and smooth running of the IPU i.e. stocks and stores management, medicines ordering, and equipment e.g. computers.

•To work with the Core Team to ensure efficient and timely care is coordinated and delivered, through coordination of e-rostering system and allocation to external agencies where appropriate in line with FH agreed specifications.

•To assist Clinical Leads with annual leave allocation and sickness cover.

•To support management in training and mentoring a multidisciplinary workforce from novice to expert to work as one team.

•To support and implement any streamlined locality/IPU processes and services by supporting the implementation of quality improvement methodology including clear outcome measures to produce efficient and highly performing community services.

•To support in building strong relationships with external partners and stakeholders and promote the good reputation of the client e.g. local trusts, CCG, health and social care partners, businesses and voluntary organisations.

•To support and guide members of the Core Team and volunteers in relevant aspects of administrative support.

Experience:
Experience in an administrative or navigational role
Experience of managing people and supporting services
Experience of maintaining and keeping accurate records
Knowledge of palliative services
Experience of managing rotas
Experience of multi-disciplinary or multi-agency team working
Experience of budget management
Experience of working within a clinical service
