Job Description
Care Co-ordinator
RESPONSIBLE TO:
Registered Manager
HOURS:
9 to 5pm and 1 to 4 on call rota including weekend
Half day on the Monday after weekend on-call
SALARY SKILL: Starting salary from GBP19000 depending on experience
Performance related addtional pay

Summary of post
To maintain continuity of care to Service Users and to ensure calls to Service Users are assigned at all times.
Experience of service provision in the care profession and must be computer literate
Experience of care coordinating within the care profession
Key Competencies

3. To be able to maintain all administration.
4. To be able to interview and select potential care staff.
5. To be able to establish and maintain effective working relationships.
6. To be able to assist with invoice and payroll queries.
7. To be able to prepare reports as required.
8. Excellent administrative skills.

10. Sound understanding of good care principles

The post holder is responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care. Under supervision of the Assistant Manager, the post holder will participate in the provision of personal and practical care to service users within their own home.

The unauthorised use or disclosures of service user or other personal information is regarded as gross misconduct and will be subject to the organisation's Disciplinary Procedure and, in the case of computerised information, could result in a prosecution for an offence or action for civil damages under the Data Protection Act 1998.
