Our Youth Housing service in Lincolnshire supports 16-17 year olds and care leavers including Young Parents up to 21 years who are homeless or are threatened with homelessness, to live independently. Our aim is to inspire the young people we accommodate to be ambitious and embrace new opportunities. Through devising a person centred support package we enable the young people we work with to have new experiences and develop skills to become independent in order to make a positive contribution to society. Who we are looking for:

We are looking for passionate, energetic and aspirational individuals to work within our Youth Housing team. Our transition worker will support Young People ready for the next step to independent living.

The ideal applicant should be patient and communicate well at all levels, have knowledge of housing options across the county of Lincolnshire as well as the benefits system.

The applicant must be flexible, have the ability to work collaboratively with others both internally and externally to the organisation in order to support out Young Persons transition to independent accommodation.

As the Transition Worker you will be working directly with young people on a 1-2-1 basis delivering person centred exit plans by supporting Young People from the point in time they are ready to move on from our Youth Housing services usually 12 weeks before they turn 18 years of age or 21 (if they are a Care Leaver)

Acting as a lead for the move on in accommodation you will take responsibility for supporting young people to apply for all available onward housing options, helping them to embed all life skills they have developed that will assist them in becoming a good tenant of the future.

You will provide advice to colleagues about future housing paths young people may follow in order they can hold meaningful discussions during their stay to prepare them for their ultimate move on from our Youth Housing service.

A DBS will be required for this role.
