Aims / Purpose of the Role:
Reporting to the Assistant Centre Manager, the Clinical Nurse Practitioner will provide support as delegated / designated in all aspects of the delivery of clinical care and support and in the operation of The Centre.
Supported by the Assistant Centre Manager and MDT members the Clinical Nurse Practitioner will co-ordinate and oversee the care and treatment of an identified group of patients and act as Named Nurse.
The Clinical Nurse Practitioner will take responsibility for the development and the delivery of evidence based practice, ensuring that it supports and reflects patient needs, The Centre objectives, internal and external KPI's and professional guidance.
The Clinical Nurse Practitioner will contribute to the development of high quality in-patient services in a changing health care environment.
The Clinical Nurse Practitioner will ensure continuous professional development through the utilisation of regular supervision, appraisal and training.
All of the Clinical Nurse Practitioners will be expected to hold an appropriate nursing qualification, hold an up to date registration with the NMC and will have additional nursing qualifications and be experienced in supporting people with within hospital or social care settings.
It is a requirement that all applicants are able to demonstrate their commitment to on-going CPD.
