Background and Service
Kaleidoscope is a pioneering substance misuse treatment provider, offering evidence based treatment and holistic support to individuals who access its services. In Powys, Kaleidoscope works in partnership with CAIS to provide services for adults and young people at tiers 2 and 3 (MoC and MoCAM). Staff teams work collaboratively across the county to deliver different aspects of these services, with a core aim of engaging substance misusers into a continuum of service provision. For individuals who require prescribing treatment, clinical and nursing services are provided by Kaleidoscope, with prescribing interventions being undertaken by South Staffordshire NHS.

Clinical services include substitute prescribing for opiate dependence, prescribing for community detoxification from alcohol, and relapse prevention prescribing for both opiates and alcohol. All clinical services are overseen by South Staffordshire NHS, with substance misuse nurses supporting the effective delivery of these services.

Clients will be offered a full range of support in order to assist them in overcoming their substance misuse; this will include harm reduction focused work delivered directly by Substance Misuse Nurses. Treatment will be individualised, and will seek to support clients to address their substance misuse as well as to raise motivation to address wider social and health issues.

Substance Misuse Nurses will deliver services in line with South Staffordshire Operational Policies and Standard Operating Procedures (SOPs), ensuring that care is safe, undertaken to a high standard and informed by up-to-date, evidence-based practice.

The post holder should enjoy face-to-face contact with people who have recognised that their use of substances has become problematic. S/he will have excellent communication skills, be able to work closely with other multi-disciplinary agencies and flexibly and efficiently within a small team, to ensure services are delivered equitably and at times to suit the client.

S/he will have a non-judgmental approach to people who have substance misuse problems and be able to provide a welcoming and accessible service to existing and potential service users.

Individual supervision will be provided six weekly for one to two hours as required. There will be an induction period, during which training will be provided in specific areas.

Key duties
The Substance Misuse Nurse will:
• Provide advice, information and guidance on the management of substance misuse to clients engaged with Powys services.
• Undertake comprehensive clinical assessments
• Assess individuals for DIP/DRR intervention and provide court and probation reports as necessary
• Carry a caseload of clients being prescribed substitute medication
• Agree treatment and care plans with clients, and review them regularly
• Undertake drug screening
• Ensure case notes and other statutory records are up to date and kept in accordance with South Staffordshire policies and procedures / regional information sharing protocols
• Deliver clinical interventions, including HCV testing, HBV immunisation and naloxone training/administration
• Liaise with Community Pharmacies to ensure prescriptions are in place and monitored appropriately
• Provide advice, support and training to GPs, pharmacists and other professionals where appropriate
• Work within the parameters of the NTA’s substance misuse community prescribing guidelines (2007)
• Provide support and cover colleagues delivering services from on-site dispensing premises where required
• Participate in clinical audits and service reviews
• Facilitate the transfer of clients from secondary to primary care services, and lead with referrals / liaison with Community Mental Health Teams (CMHT’s)
• Work with other regional treatment services to support the development of the local treatment system, and in particular Shared Care services
• Actively encourage service user involvement and feedback and ensure views are considered in informing the development of the service.
• Collate and enter data electronically, and contribute to service reports
