Hospital Assessment Team (HAT)

We are a social work service based within a hospital, working with patients that are admitted to hospital, as well as their families and carers. Join us, and, together, we'll deliver social care as it should be. Here, you'll have the time to assess, plan and support service users to cope with social, emotional, economic and environmental problems. Ultimately, you'll share our focus to help service users to maintain their independence and to live as normal a life as possible in the community. This will mean working in partnership with health staff on social matters that may affect present and future health. You'll also have our full support in completing safeguarding investigations and continuing healthcare assessments. It's all about working as part of a multidisciplinary team, and you will draw upon domiciliary, day and residential services to support service users on discharge from hospital.

Knowledge, skills and experience
  • Diploma / Degree in Social Work, CQSW, CSS or equivalent or Diploma of the College of Occupational Therapists or B.Sc. O.T. or other professional qualification recognised by the World Federation of Occupational Therapists HCPC registered
  • Case load management in line with the Care Act
  • Excellent organisation skills with the ability to manage your own time
  • Ability to engage and influence with multiple agencies
  • Passionate and driven to support your own continuing personal development (CPD)

The role requires you to be mobile throughout a wide operational area including travelling around the county of Essex and beyond, transporting service users and their belongings and to be able to do so in an emergency. Driving licence and car or ability to meet the mobility requirements of the role through other means is essential.

As part of employment with Essex County Council should your role require a DBS check you will be required to register with the DBS update service. The DBS update service ensures all registered DBS certificates are kept up to date and allows authorised parties to view the status of your certificate.
