They are currently looking for a Care Coordinator to join the steady growing and enthusiastic team to assist in the management the scheduling of those client visits. With the business growing over 30% in the past year, they are looking to add more professionalism to the group.
Job specificity:
- Permanent allocation of Care & Support Workers to all Customer visits, ensuring all visits consistently covered.
- Effective communication with Customers, Care & Support Workers and the management team.
- Accepting and scheduling new Customer referrals.
- Maintaining customer and Care & Support Worker office files, ensuring consistence & compliance
- Responding to Customer changes of need
- Preparing appropriate documentation for Field Care Supervisors.
- Ensuring all other records are kept up to date.
- To ensure compliance with all office systems and procedures.
- Work collaboratively with Managers, Supervisors and Care staff
- Prepare management reports as required
- Be responsible for risk assessments, taking on new clients, introduction of care givers to clients and hands on training
We are looking for someone who:
- Is highly organised and can plan multiple projects at one time
- Must have experience and good knowledge of scheduling for a number of carers (knowledge of people planner is preferred)
- Is able to work and stay calm under pressure
- Is willing to be on call 1/3 weekends
