Under the Community Links for Health (CLFH) funded programme, some exciting opportunities have arisen at this organisation.
As one of four new Community Link Workers, you will be working closely with CLFH teams at both 'hub' and 'neighbourhood' levels. Referrals will be made into the service from G.P's and other health and social care professionals; clients will be assessed at the 'hub' and referred to the appropriate support agency including this company. Your role will be to assess the client's support needs and arrange appropriate person and community based intervention, whether it is using your own knowledge and experience or from utilising partners and relevant organisations within the community.
You will liaise closely with all CLFH partners and the wider company teams to provide the required interventions, whether that is signposting to appropriate community based groups and activities, helping to improve health and wellbeing, motivation, confidence and offer information, advice and guidance specifically relating to housing needs and options.
The role will be located across a number of CLFH delivery locations in north Manchester, with main locations being at local housing offices of both the company and One Manchester. As such, you must have your own transport and be able to work flexibly. You will be expected to manage a caseload of referred clients, with each person referred into the Community Link Workers receiving low-level support (6-hours of support on average).
If you have great communication and interpersonal skills, are self-motivated, highly organised, have excellent prioritisation skills and passionate about delivering a first class customer service then apply today!
