South Quadrant Children and Young People with Disabilities (CYPWD) are currently running a pilot for the 0-25 team meaning we work with children and young people aged between 0-25 who meet the criteria for intervention from the CYPWD team.

The 0-25 service for children and young people with disabilities and/or additional needs brings children and adult social workers together to provide a single service to support a young person from birth until the age 25, avoiding the need for a formal transition at the age of 18.

The role of the social worker in the young people with disabilities team is to manage a caseload of children/young people with disabilities requiring assessment of their needs, advice, support and development of care plans and support packages in accord with practice guidance.

Within your team you will provide an integrated service to help families meet the needs of their children/young adults, build their resilience and support young people progress from childhood into adulthood.

Please note that for this team, Adults experience is particularly desirable.
