The Role

Primarily to facilitate and support people to undertake person centred, asset focussed assessments and reviews. This work is outcome focussed and driven by the principles of the Care Act. This approach will help each person to reach their potential and connection to the networks and community around them.

About the Increasing Independence Team
The Increasing Independence Team has two main work streams:
1. Supporting individuals to move from Residential care to Supported Living as tenants. As explained above ensuring this support maximises independence and enables each person to develop.

2. Reviewing and rationalising collective and individual programmes in Supported Housing, again maximising opportunities for growth and increased independence.

Knowledge, skills and experience
What we need from you
?? There is a requirement to build relationships with tenants/prospective tenants, their families, staff in residential care and colleagues in community teams and other service providers.
?? Diploma/Degree in Social Work, CQSW, CSS or equivalent or Diploma of the College of Occupational Therapists or BSc OT or other professional qualification recognised by the World Federation of Occupational Therapists.
?? Registration with the Health and Care Professions Council.
?? A good working knowledge and ability to use information technology and related systems.
?? Must have two years post qualified experience.
?? Approved Social Worker status (specific to Older Adult Mental Health).

The role requires you to be mobile throughout a wide operational area including travelling around the county of Essex and beyond, transporting service users and their belongings and to be able to do so in an emergency. Driving licence and car or the ability to meet the mobility requirements of the role through other means is essential.

As part of employment with Essex County Council (ECC) should your role require a DBS check you will be required to register with the DBS update service. The DBS update service ensures all registered DBS certificates are kept up to date and allows authorised parties to view the status of your certificate.
