Our client's in Preston, Lancashire are recruiting for an Learning Disability Support Worker - with proven experience of providing personal support and care to individuals who have complex care and learning disability needs with the aim of promoting maximum independence, responsibility and control over their own life. As a Support Worker will contribute to the development of the person's self-confidence and social skills and explore positive opportunities to access community life. Working as a member of the Supported Living team the worker will develop appropriate individual programmes to meet the individual plans of service users.

Qualification – Essential
Reasonable general education (evidence of numeracy and literacy)
Level 2, 3 or 4 NVQ in Health & Social Care or an equivalent qualification is highly desirable
Applicants without a social care qualification will need to prove competencies to be considered

Skills, Abilities & Experience – Essential

Proven and recent experience of support individuals to access agreed programmes and personal plans

Experience of accessible recording of individual's activities and personal development

Experience of acting as key worker to individuals to ensure their personal plans are delivered, monitored and evaluated

Knowledge of personal planning and review meetings

Knowledge of developing age appropriate activities in the wider community

Experience of assisting with personal care needs and contribute to regular health care assessments

Strong knowledge of administer procedures to meet the complex health needs of individuals (

Must be willing to undergo continuous training and development

Proven and recent experience of working closely with family, carers and other professionals

Proven Advocate alongside the person to promote their views

Able to promote positive practice of Safeguarding, Health & Safety and Mental Capacity within the workplace

Strong ability to maintain accurate and up to date records and assist in the monitoring and evaluating of the individuals' service provision and plans
Able to work flexible hours – likely to including sleep-in per week and some weekend
Knowledge and commitment to safeguarding and promoting the welfare of vulnerable people
Understanding of the Supporting People agenda and Quality Assessment Framework standards
Understanding of Social Care legislation relating to learning disability including:
Valuing People / Our Health, our care, our say / Mental Capacity Act
Registered Enhanced/Standard DBS Disclosure within the last 12 months
Understanding and commitment to the Equal Opportunities policy
Must be able to provide references covering the last 5 years
