Our clients vision is that everyone has a place to call home and can fulfil their hopes and ambitions.
They have received funding to deliver a new No First Night Out, Homeless Prevention service across West London. The service will work towards ensuring clients at risk of rough sleeping, are brought off the streets, as well as provide an assessment hotline for referrals and triage. The new service will also have lead workers in all boroughs across West London, assessing NFNO clients, sustaining clients off the streets and enabling them to reach their life goals.
You will be responsible for delivering, a rapid assessment service to assess people at risk of rough sleeping; either by phone or email. You will do this in partnership with your colleagues, day centres, outreach teams, local authority and other partners.
You will also to provide clients with personalised case management support to help them remain in their home, if it is safe to do so, or to access alternative accommodation. Where rough sleeping cannot otherwise be prevented, place clients in the "Safe Space" hub and provide support and access move-on accommodation.
To succeed, you will have personal experience of homelessness OR a sound understanding of the issues faced by homeless or vulnerably housed people. You will also have experience of supporting people to address their needs through case management and an understanding of the importance of involving clients fully in the process.
Knowledge of the welfare benefits and housing law the range of drug and alcohol services appropriate to single homeless people, is also essential. As is strong communication and negotiation skills and the ability to build strong partnerships with external agencies
