An opportunity has arisen for a motivated, dedicated and passionate individual in the position of Home Care Co-ordinator within our home care service. Supportive provides domiciliary care to vulnerable people in the community and this is a key role in the organisation.
The Home Care Co-ordinator must work in line with our policies, procedures, code of conduct and Core Values, as well as current legislation. The successful candidate will:

Knowledge and experience of the domiciliary care sector and geographical area of County Durham is preferable. The position will be conditional upon a satisfactory DBS check and holding a full, clean driving licence.
Key roles and responsibilities:
- Liaise with commissioners of services such as Local Authority, NHS and private referrers to accept and allocate packages of care.
- Develop and maintain strong relationships with care workers, customers and their representatives liaising with them to allocate appropriate schedules of care.
- Coordinate and distribute care worker rotas and customer schedules, communicating with senior support workers to ensure implementation of care plans.
- Accurately update the current database within organisational timescales.
- Audit customer documentation, addressing issues raised and documenting outcomes within organisational timescales.
- Assist with recruitment of care workers as required.
- Actively participate in the out of hours’ service for home care on a rotational basis to provide advice and guidance to all stakeholders when head office is closed.
