Our client’s in West London are recruiting for an Independent Living Assessor- Adult Social Care - with proven experience of providing timely and responsive assessments, re-assessments and/or reviews delivering personalised care to support users and carers with non-complex needs to shape their own lives, encouraging maximum independence and safety.

Qualification – Essential

NVQ level 3 or equivalent qualification or academic standard Or Evidence of achieving a relevant

Or

Demonstrate a minimum of 5 years successful relevant experience of undertaking assessment of need, completing support plans and providing services to a range of client groups including people with complex needs.

Skills, Abilities & Experience – Essential

Knowledge and understanding of the issues facing people who may need Social Care and Community Equipment Services

Relevant experience of working with an Adult Social Care user group

Experience of working routinely with electronic recording systems– and excellent recording skills (precise/concise/relevant)

Excellent standard of numerical and verbal comprehension and reasoning (Including the ability to search and identify different types of provision and their costs and to make reasoned recommendations)

Excellent communication skills (precise /concise / relevant)

Ability to advise, instruct and as relevant demonstrate to service users, carers, family representatives, liaise

Knowledge of the basic concepts of NHS &Community Care Act, Health &Safety legislation and guidance pertaining to stability equipment provision and duty of care.

Ability to undertake assessments and organise follow on services, equipment, and adaptations

Proven hands on and recent experience of: Chronically Sick and Disabled Persons Act 1970, National Health Service and Community Care Act 1990, Data Protection Act 1988)

COT Code of Ethics and Standard of Practice GSCC Code of Practice)

Case Recording, Case File Management, Data Recording Requirements, Health and Safety, Lone Working)

Personalisation Programme, Integrated Care Pilot)
