Transform Facilitator

Queenscourt Hospice, a local charity providing support for people of Southport, Formby and West Lancashire.

We are recruiting for Transform Facilitator.

• £26,829 - £35,931 pro rata
• Part Time, 12 month
• Fixed term (contract extension dependant on funding)

Working as part of the Transform Team to educate and improve the care of those in the last months of life in hospital, care homes and at home by educating and supporting clinical staff, families and patients. The successful candidate will be a registered nurse with previous senior level experience of end of life care. The post-holder will be expected to work their hours over seven days a week.
