Discharge Nurse

 

HFH Healthcare is a complex healthcare service providing bespoke packages of care to highly dependent adults & children living at home with complex conditions requiring specialist care.

 

Our aim at HFH Healthcare is to provide our patients with a care team of the highest calibre. Our team of qualified specialist nurse trainers train all selected candidates in our fully equipped specialist training facility and further support all care teams in the patient’s home on a daily basis.

 

The Discharge Nurse is a key role within the Clinical Services Team, supporting the business with the management of on-boarding new patients from referral through to discharge in a timely manner.  You will be responsible for co-ordinating all the clinical tasks required to transfer the patient home safely. 

Working closely with the clinical nurses, you will manage the full process from arranging initial clinical assessments through to commencement of care within agreed deadlines, stabilisation of the care package, and liaising with nursing and allied health professionals, keeping them informed throughout the process.

 

The Discharge Nurse will need to have close co-ordination with the Operations team, specifically around ensuring carers are identified prior to discharge and are appropriately trained to deliver the care.

 

To be successful you will need to have:

 

· Nursing qualification with current Registration with the NMC.

· Previously undertaken holistic clinical assessment skills, including comprehensive risk assessments, and clinical reasoning skills (ideally).

· An organised, quality focused and positive approach to work with good attention to detail.

· Experienced in building relationships with a wide range of professionals.

· Strong interpersonal skills, able to establish rapport quickly and able to motivate people to make things happen.

· Proven ability to analyse complex problems/information from a variety of sources to inform decisions and solve problems.

· Resilience and able to work under pressure, maintaining high quality standards.

· The ability to work collaboratively and effectively as part of a team.

 

Pay Rates: Annual Salary £40,000                 

Hours: 37.5 (office based in Wimbledon with some travel required through public transport and the use of a pool car, therefore full driving licence an advantage).

Annual Leave: 25 days plus bank holidays
