Salary from 30,000 - 35,000 per annum on appointment with performance related progression up to 40,750

We are a County-wide team working creatively alongside adults and some young people with disabilities to maximise their independence and opportunities to live in non-institutional settings at home and in their communities.
Either:-
- Supporting individuals to move from Residential care to Supported Living in the community.
- Reviewing and rationalising existing Supported Living Schemes, focussing on collective and individual support required.
- New challenges within day opportunities
We require enthusiastic social workers committed to undertaking person centred work with individuals focussing on their strengths, interests and assets in accordance with the Care Act. You will be passionate about developing a clear and comprehensive understanding of what is most important to each person to ensure they lead a valued and good lives. This understanding will mean the Care Act Assessment/Review will provide a clear picture of how a person's independence can be maximised within a Supported Living / community setting to meet their needs. The assessment will draw on the support around each person and their local community. It will also where necessary recognise and understand potential risks and identify specific person centred inputs required.
You will be provided with in-depth supervision, training and be encouraged to take advantage of creative peer support opportunities. You will be encouraged and supported to work independently and therefore use your initiative and form grounded opinions and judgements within an experienced and dynamic team.
