LOCATION: Frome, Yeovil, Chard, Taunton, Bridgwater, Burnham-on-Sea, West Somerset

JOB TITLE: Reablement Worker

SALARY & BENEFITS: GBP8.40ph- GBP9.20 per hour

WORKING HOURS: 39 hours per a week- contracted hours, to include alternate weekends

THE COMPANY: We are pleased to be working with our client who is a not-for-profit care provider dedicated to providing high-quality, personalised support to residential and nursing care for older people in 30 care homes in Somerset and on the Isle of Wight. An exciting opportunity has arisen for Reablement Workers to really make a difference to individual's lives in the Somerset area.

Do you have a passion for supporting people to lead independent lives? Are you qualified to a level 2 in Health and Social care or equivalent? Working in the either Frome, Yeovil, Chard, Taunton, Bridgwater, Burnham-on-Sea, or West Somerset area, you will develop outcome focused support plans to accurately reflect the achievements and provide a range of care and support tasks to individuals living within their own homes during the 6 week reablement assessment period.

Key Responsibilities -

- Adheres to the initial outcome focused support plan for each new Customer as directed by the Community Team Supervisor, ensuring Customers personal aspirations and wishes are taken into account and where necessary responding with initiative to complete this task.

- Physical assistance Support with mobility

- To sustain independence within their home

- Undertaking care and support activities and identifying other services or equipment which will meet the needs of the individual Customer and promote their independence and re-enablement.

- Undertakes continual assessments throughout the assessment period, communicates progress reports to the Community Team Supervisor regarding the needs of individual Customers.

We are looking for great people to join the Reablement Worker team. You will have previous experience within a care setting and hold a minimum level 2 in Health and Social care or equivalent.

Key experience essential for the role:

- Previous experience in a care setting

- Qualified to level 2 in Health and Social Care or equivalent

- Empathy, caring and sensitivity to the needs of others

- Driving licence

- Happy to work full-time, alternate weekends and can do shift patterns
