Complex Care - Referrals Coordinator

HFH Healthcare is a complex healthcare service providing bespoke packages of care to highly dependent adults & children living at home with complex conditions requiring specialist care. Our aim at HFH Healthcare is to provide our clients with care teams of the highest calibre.

We are looking to recruit a dedicated Complex Care - Referrals Coordinator to work closely within the Care team to effectively set up new cases in a timely manner ensuring they are appropriately staffed and monitored to ensure they become stable packages within a set deadline. Our in-house clinical team of nurses provide the specific training; however you will need to ensure the care team are appropriately allocated to match their skills and experience.
Reporting to the Referrals and Discharge Manager you will be responsible for the implementation of new care packages. Ensuring that the patient and customer is kept up to date on the progress is key. Providing detailed work schedules for the healthcare assistants, providing both written and verbal instructions / guidance to them. This role will be the first point of contact for the patients, customers and care team for new referrals.
This is a challenging role in which you need good organisation and administration experience to handle the challenges which arise on a daily basis. However, it is a rewarding job where you will have the opportunity to make a positive difference to the service we provide to our patients.

To be successful you will need to be solution focused, able to deal with a fast-paced environment, working under time pressure. You will be responsible for dealing with any changes to the rota ensuring sufficient cover for your client is provided at all times.

Ideal candidate:

· Complex Healthcare coordinator experience
· Ability to manage a team of healthcare workers
· Experience of managing staff rotas
· Experience of working with a database/CRM system
· Able to work under pressure and work to tight deadlines
· Able to meet and exceed set targets
· Able to multi-task and be organised
· Resilient and good interpersonal skills
· Ability to deal with last minute changes


Pay Rates: Annual Salary £26,000 pa.

Hours: 37.5 (office based in Wimbledon with occasional travel).

Annual Leave: 20 days plus bank holidays
