Our client is a leading mental health provider in Bristol, North Somerset, B&NES and South Gloucestershire. Their aim is to deliver recovery and wellbeing opportunities for people with mental health and other support need to achieve their hopes and ambitions.

Facilitated Discharge Worker - Recovery Bristol Partnership

Salary range: £20,369 - £22,265 per annum

Hours: 37 per week

We have a full time position, for 37 hours per week, on a permanent basis.

The successful candidate will be responsible for acting as the link for inpatient wards, community teams and supporting safe discharges, while maintaining a safe level of risk.

You will also liaise with service users and ward staff prior to discharge to identify potential barriers and reduce the length of admission.

You must have a minimum of two years experience working within Mental Health.
