An exciting opportunity has arisen to join a dynamic hospital discharge service that works with customers across the London Borough of Hammersmith & Fulham, Royal Borough of Kensington and Chelsea and Westminster City Council. We are looking to appoint an experienced, enthusiastic, energetic, and motivated manager / senior practitioner to join the dynamic team and manage a team of social care practitioners based at Charing Cross Hospital. Your team will work collaboratively and proactively with our Acute colleagues to sucessfully discharge patients from hospital as part of a multi-disciplinary team.

Operating on site in a hospital, the successful applicant will be working as part of a vibrant service offering day to support to the team, dealing with challenging hospital discharges, and will be expected to offer expert social care input and be responsible for the overall management of the team;

• You will have a recognised social work qualification and will be a professionally qualified, registered social worker adhering to the Health and Care Professions Council (HCPC) standards of proficiency
• You will be an experienced manager / senior practitioner, who has worked in complex disability/discharge fields (or cases) and who is committed to a multi-disciplinary approach
• You will be a can do person who is able to think creatively to resolve issues within a whole systems framework
• You will be a problem solver, supporting your team to respond proactively and efficiently to increasingly complex discharges and collaboratively working with health colleagues
• You will be able to cope under pressure and work to tight deadlines
• You will have excellent verbal and written communication skills
• You will be well informed of the Care Act (2014) rquirements which should underpin day to day practice
• You will work in an asset based approach promoting independence and choice always
• You will be experienced in completing Mental Capacity assessments with high standards, committed to Safeguarding adults who are in need
• You will have good understanding of the carers rights under the Care Act (2014) and ensure they are supported throughout the process
• You will have strong interpersonal and organisational skills
