Overview
Our Adult Social Care Service are currently recruiting for positions in the Learning Disability, Transitions, and Continuing Health Care Teams.

About us
We aim to support people with a learning disability to live a normal life in a community they can be a part of. We focus on person centred practice to enable people to stay safe; make choices about how they live their lives; and help them get the care and support they need. We work in partnership with users and families, providers and health colleagues, often for many years.

About the role
In this key role you'll work with adults, their families and support networks, or adults with learning disabilities, supporting them through changes in their lives and helping to build their community networks. You'll complete assessments; identify people's outcomes and care and support needs; plan care with individuals, families, brokers and providers; and review packages to check what is working well and what needs to change. You'll need to develop a working knowledge of the Care Act, Mental Capacity Act, and Mental Health Act; and the skills to be able to apply them to casework. Working closely with a wide range of health, social care, contracts and provider colleagues to keep people safe, meet their eligible care and support needs, you'll develop and use your knowledge; professional experience and reflective practice to ensure that key concepts and theories in social work are used to ensure the best possible outcomes. Working within a team with shared responsibilities, you may take on supervisory, development and coaching roles for other staff. As you become more experienced our senior posts include additional responsibilities including complex casework (eg. court work, forensic, complex disability and tribunals); practice educator roles; leading areas of responsibility (eg. Safeguarding, behaviour that challenges, duty, reviews); and deputising for the business manager. Please note that there are positions available in Aylesbury, High Wycombe, and Countywide. About you
Familiar with legislation for adults and able to apply it to practice you'll be proactive in developing your knowledge and skills and sharing them. With recent experience in a health or social care setting you will be able to develop excellent assessment, report writing and organisational skills. An excellent communicator with great interpersonal skills, you will work in partnership with people with diverse needs, both service users and carers; and colleagues and providers. Caring and collaborative, you'll be motivated and reliable, flexible and resilient. You will be able to organise your own time and be able to work to deadlines. A recognised health and/or social care qualification, a car user, and registration with the HCPC/NMC is an essential requirement for this role.
