Mae Prosiect Gofalwyr Ifanc  Gwynedd, Gweithredu dros Blant – Action for Children yn darparu cefnogaeth i blant a phobl ifanc sydd â chyfrifoldebau gofalu. Mae gofalwyr ifanc yn wynebu trafferthion sylweddol a than bwysau parhaus sy’n effeithio ar bob agwedd o’i   bywydau. Nod y prosiect yw gwella ansawdd eu bywyd, datblygu gwytnwch a’u cynorthwyo i gyflawni eu potensial. 

Gwynedd  Young Carers Projects, Gweithredu dros Blant - Action for Children provides support for children and young people with caring responsibilities. Young carers face considerable difficulties and under constant pressure which affects all aspects of their life. The project aims to improve their quality of life, build resilience and help young carers achieve their potential.

How you’ll make a difference:

Rydym yn chwilio am rywun sydd eisiau gwneud gwahaniaeth ym mywydau gofalwyr ifanc drwy ddarparu cefnogaeth ar sail unigol a hefyd mewn sefyllfa gwaith grŵp. Bydd angen iddynt allu cyfathrebu’n effeithiol gyda phlant, pobl ifanc a’u teuluoedd er mwyn cyfarfod a’u hanghenion yn cynnwys: eu hiechyd a’u lles, addysg a chyflogaeth, materion yn ymwneud â pherthnasoedd ac ymddygiad, ymdrin â gwahanu a cholled, cefnogaeth ymarferol.

Bydd yr ymgeisydd llwyddiannus hefyd yn meddu ar sgiliau asesu da, yn gallu gweithio ar y cyd â chydweithwyr o asiantaethau eraill er mwyn darparu'r gwasanaethau mwyaf priodol ac amserol i deuluoedd er mwyn creu newid cadarnhaol. Mae adolygu a monitro effaith ein hymyriadau hefyd yn rhan ganolog o'r swydd hon.

Mae diogelwch y plant, y bobl ifanc a’r teuluoedd yn hollbwysig a bydd yr unigolyn llwyddiannus angen gwybodaeth dda a dealltwriaeth o ddiogelu.Er mwyn darparu gwasanaeth ymatebol, mae disgwyl i’r gweithiwr allu blaenoriaethu ei llwyth gwaith, gweithio'n annibynnol ac ymateb yn hyblyg.                  

We are looking for someone who wants to make a difference to the lives of young carers by providing support on an individual basis and in group work situation. They need to be able to communicate effectively with children, young people and their families to meet their needs include: their health and welfare, education and employment, issues of relationships and behaviour, dealing with separation and loss, practical support.

The successful candidate will have good assessment skills, able to work in collaboration with colleagues from other agencies to provide the most appropriate and timely services to families in order to create positive change.  Being able to review and monitor the impact of our interventions are also an integral part of this job.

The safety of the children, young people and families is crucial and the successful candidate will require a good knowledge of the individual success and understanding of safeguarding.  In order to provide responsive service, the employee is expected to be able to prioritise its workload, work independently and respond flexibly.

  • Trwydded yrru lawn a defnydd o gludiant dibynadwy gan fod y swydd hon yn golygu gweithio ar draws Gwynedd
  • Mae’r gallu i ddefnyddio cyfrifiaduron yn hanfodol yn y swydd hon
  • Dealltwriaeth o’r materion a wynebir gan ofalwyr ifanc a chan deuluoedd bregus
  • Bydd y swydd hon yn golygu gweithio yn ystod y dydd a gyda’r nos, gan gynnwys rhai penwythnosau.  Ar adegau efallai bydd yna angen i weithio yn Ynys Mon.
  • Good standard of education that will enable you to complete assessments, record keeping, monitoring reports to partners and commissioners
  • Health and Social Care qualification or equivalent is desirable
  • Experience working with children, young people and their families
  • Understanding of child development
  • Being able to work through the medium of Welsh is essential
  • Full driving license and use of a reliable transport since this job involves working across Gwynedd
  • The ability to use computers is essential in this post
  • Understanding of the issues faced by young carers and vulnerable families
  • This post will involve working during the day and evening, including weekends. On occasions there may be a requirement to work in Anglesey
