The Essence Service is a joint initiative between Age UK Sunderland, Sunderland Carer’s Centre and Sunderland CCG to provide ongoing support to people with Dementia and their Carers to enable them to have a full and active life in the community.
We seek a self-motivated and enthusiastic individual to provide one-to-one support for people with dementia and their carers. Excellent verbal and written communication, and IT skills and experience/knowledge of working with dementia are required. Access to a car for work use is essential.
The Essence Service Support Worker will provide practical, social and emotional specialised support to individuals who are living with a diagnosis of dementia and their Carers.
The successful candidate will:
- Ensure the provision of innovative interventions and continuous practical, social and emotional support to those living with a diagnosis of dementia and their Carers.
- Develop a personal plan for the person with dementia, which looks at what they would like to do and what the barriers are to maintaining their quality of life and independence. Develop a personal plan for their Carer to include the carer’s needs in relation to their caring responsibility and their own personal needs and development.
- Manage an active caseload.
- Act as a navigator and a listening ear that many people with dementia and their carers need.
- Promote a self-management approach which will look to empower people with dementia to help themselves through peer to peer support and access to move specialised service e.g. financial planning and legal services.
- Actively maintain the ‘Essence’ of the person with dementia – whether that be in the hobbies they enjoy, being able to do everyday activities for as long as possible in their home and community.
- Encourage and support the person with dementia to move into mainstream services, which can accommodate their needs and interests.
- Build a peer network that is supportive and offers opportunities for self-help and the sustainability for the person with dementia and their carer.
- Support the person with dementia’s carer to access support and advice and make known their life choices about their future care.
- Work closely with Sunderland Carers Centre Support specialists.
- Actively promote the service.
- Support the recruitment, training, and develop volunteers involved in the delivery of the Essence Service.
- Keep records of clients and volunteers.
