CHC Nurse Assessor

5 weeks contract, with extension for 6 months

Location - West Yorkshire, near Huddersfield

Rate - GBP200 - GBP249 per day

We're looking for a CHC Nurse Assessor to join a flourishing healthcare team in the West Yorkshire area. Are you an analytical thinker with outstanding communication skills looking for the next exciting step in your career? If so, this may be the perfect role for you.

What will you be doing?

As CHC Nurse Assessor, you will be responsible for facilitating multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures.

You will deploy a thorough understanding of the Continuing Health Care National Framework and its guidance and processes. You will also be responsible for managing your own case load as designed by the Clinical Team Leader. You will demonstrate meticulous attention to detail when investigating individual eligibility for Continuing Healthcare funding and will display excellent communication skills when working alongside other agencies and providers collaboratively.

Who should you be?

• A RGN with a current PIN from the NMC
• Able to deal 1-2 live cases per day
• CHC Checklist and DST experience
• A professional with experience of working effectively with clinicians, social care and other partner agencies
• Carry out assessments in varying locations such as residential homes, nursing homes and care homes.
• A candidate with experience using NHS framework and have 6 months experience of CHC
