Supported Discharge Worker
An opportunity is available to work within the new Age UK Stockport Hospital
Discharge team providing high quality person centred care and support to vulnerable people including older people to enable them to be discharged from hospital / care setting in a timely and safe manner, by providing practical care and support within the person’s home and / or within the wider community, which will prevent or reduce the need for re-admittance.
Hours are available from 18.5 up to 37 hours per week.
