The Adult Social Care Integrated Triage Hub provides a multi-professional screening service for all referrals received by Bexley ASC using a 'see and sort' solution-focused model of intervention. It provides a Duty response for the Complex Care Teams and CART. Working closely with Health colleagues and the Rapid Response Team, the Triage Hub will deal with crises and emergencies as they arise.
We are looking for a qualified Social Worker with a firm grasp of Adult Safeguarding and of working in a multi-disciplinary environment. You should have a clear understanding of the Care Act and of the national agenda on the integration of Health and Social Care. You should be able to demonstrate a firm commitment to person-centred working and have a 'can do' approach to day to day challenges.
You'll work as part of a multi-disciplinary team, assessing the service user's needs (and that of their carer), to come up with a creative, yet practical, programme that will enhance their quality of life.
We're looking for a qualified HCPC registered Social Worker with post qualifying experience, having completed your AYSE award (if applicable). However, this role may also be suitable for a talented and dedicated Newly Qualified Social Worker. The most important things are that you genuinely enjoy working with vulnerable adults and can demonstrate your capabilities against the Social Work Professional Capabilities Framework (PCF).
You'll be flexible, approachable and adaptable - towards service users and your colleagues alike - as well as being someone whose expert professional and legal knowledge is backed by a human touch. You will also be able to demonstrate experience in conducting Safeguarding Vulnerable Adults investigations. You may also be a qualified Best Interests Assessor or Practice Educator or be prepared to complete training in at least one these areas.
Appointment to this post will be subject to the outcome of an Enhanced Disclosure obtained through the Disclosure and Barring Service.
